Medicine Unit

Department of medicine is one of the leading clinical departments in hospital. It provides services which include Clinical duties, Teaching and Research through its various units, manned by highly skilled Consultants, and resident doctors in training.

UNITS OF THE DEPARTMENT

Respiratory Unit (Chest)

Summary of activities of each dept (see attached tables)

The unit ran two medical clinics per week:

1. OUTPATIENT CLINIC:

Monday - Respiratory Clinic. This is when specialized medical chest conditions are seen, which are referred from with and outside the hospital.

Wednesday - General Medical Clinic. Here general medical problems requiring physician's attention are seen.

2. WARD ROUNDS

•  House officer's daily ward round

•  Residents' ward round: this takes place on Fridays.

•  Consultants' ward rounds and teaching round takes place on Tuesdays and Thursdays.

CALL DUTIES

The unit is on call on alternate Wednesdays as well as assigned weekends.

TEACHING

•  Medical Students: the unit was involved in the teaching of various categories of Medical Students in the out going year. Students were in postings and had lectures, tutorials and clinical demonstrations.

•  Resident Doctors: The Consultants in the unit organized teaching sessions for the resident doctors in the department.

How we run the clinic.

Mondays - General Medicine Clinic; Unit Seminar 2-3pm

Tuesday - Consultant ward round

Wednesday - Echocardiography

Thursday - Specialty Cardiology Clinic.

Friday - Consultant ward round.

Research Forum 1-3pm

Research Interests: - Blood pressure Control in Hypertensive patients.

Audit of Echocardiography in LASUTH 2002 - 2006

We see about 70 patients in each clinic. We usually have about 20 in patients on an average. Our calls come up every Monday.

 

Dermatology Unit

The unit is located at the extreme end of the Medical Outpatients' Department.

It occupies a 2room air-conditioned consulting suite with a large waiting room and a mini theatre. The Unit attends to about 30 new cases per week on Tuesdays and about 30 follow-up cases weekly on Wednesdays . All clinics are by prior appointments but emergencies are given due consideration . Clinical procedures are done on Thursdays . The unit is ‘On Casualty Take' on Wednesdays when all emergencies of whatever specialty are received via casualty. Admission is as per available beds anywhere in the Medical Wards.

 

Gastroenterology

Dr C. A. Onyekwere MBBS, FMCP, MEMBER ASGE - Chief Consultant Physician and Gastroenterologist

The Gastroenterology unit is headed by a consultant with a complement of one resident and one or two house officers depending. It maintains/runs an endoscopes unit with the help of a private entrepreneur under a government public - private sector arrangement. The unit has 1 Gastro scope, 1 colon-scope with a camera and monitor. Recently I unit used video-colonoscopy and sigmoid-scope were donated to the unit but these are yet to be put into use because of some technical problems with the equipments. At the moment the unit can afford to offer diagnostic upper and lower G.I. Endoscopy.

RESEARCH

The unit concluded a drug trial commenced fast year during the 2006; the efficacy of Livolin forte (phosphatidyl choline) in patients with chromic Liver disease. This was presented at the meeting of the association for the study of the Liver in Nigeria (ASLIN) in Jos.

Also the following research works were concluded;

•  the prevalence of Gastrointestinal symptom in Diabetics.

•  The upper G.I Endoscopy findings in Lagos, Nigeria.

•  Prevalence of irritable bowel syndrome.

 

Neurology Unit

Neurology Teaching Rounds/Call - Mondays/Thursdays

There are two Consultants in the Unit assisted by resident doctors and house officers. The daily unit activities are as enumerated below:

MONDAY - 9.00am - 2.00pm

Consultant ward rounds on all medical patients on admission Bedside Clinical Teaching of residents and medical students.

TUESDAY 9.00am - 3.00pm

Neurology outpatient clinic on all neurological cases referred from within and outside the hospital.

Teaching/Clinical demonstrations to residents and medical students.

Fortnightly departmental consultant meeting at 1.00pm, where issues pertaining to the department are discussed.

WEDNESDAY 9.00am - 1.pm

Consultant ward rounds on all medical patients on admission. Also involve in, Clinical teachings demonstrations to residents and medical students.

Weekly Departmental Clinical grand rounds, during which the unit presented a number of neurological topics of interest.

THURSDAY: The unit call duty day (7.30am to 7.30pm next day). The unit was on call on alternate Thursdays.

Resident ward rounds (Mornings) on all patients on admission.

FRIDAY 9.00am - 1.00pm

General medical outpatient clinic is where all general medical cases requiring physician's attention are attended to. Also are Clinical teaching, demonstrations to residents and medical students.

The unit also ran weekend call (48 hours) as assigned on the monthly call duty rosters.

As a routine the house officers attended to all patients on admission on a daily basis.

The unit Consultants gave lectures as well as tutorials sessions to medical students of different levels based on the departmental lecture and tutorial schedules.

 

Endocrinology Unit

SCHEDULE OF ACTIVITY

The unit schedule of activity is follows:

Mondays - Ward rounds/G.I procedures.

Tuesday - Clinics (Specialist G. I. Clinics)

Wednesday - Endoscopy and other G. I. procedures.

Thursday - Clinics (General Medical Clinic/Call duty.

Friday - Consultant teaching Rounds.

Weekend calls every 7 weeks.

CLINICAL SERVICES

On the average we saw an average of 10-20 new patients and 70-100 old follow-up patients in the clinics every week. The unit admits inpatients from the clinics and the Emergency Medical Dept into the ward.

On the average we admitted about 5 – 10 patients every weeks. These inpatients are reviewed regularly beside out routine ward rounds depending on the situation.

Endoscopy services (diagnostic upper and lower G.I) and other specialized G.I procedures e.g. Liver Biopsies are carried out on demand. But routinely endoscopies are done on Wednesdays average 4-6 scopes a week.

ACADEMIC ACTIVITY

The unit is very much involved in teaching activities involving both undergraduate and post-graduate students. The academic activity involves; case – Gastroenterology lectures for the undergraduates.

•  Seminars for the residents

•  Bedside, Clinic and Ward round teachings

•  Presentation at departmental and hospital grand rounds. During the year, the following topics were presented at these grand rounds;

•  Inflammatory bowel disease, case presentation.

•  Non-alcoholic fatty Liver disease a case presentation.

•  Gastrointestinal Endoscopy; LASUTH experience.

RESEARCH

Nephrology Unit

CLINICAL ACTIVITIES

1. Ward Rounds - This is on regular bases.

2. General Medical Clinics:

•  We have regular general clinic on Fridays. Patient load varied between 20 to 30 at each of the clinics.

•  Averages of about 7 new cases were seen per clinic.

3. Subspecialty Clinic:

•  We have regular sub specialty clinic on Wednesdays. Patient load varied between 20 to 30 at each of the clinics.

•  An average of about 5 new cases was seen per clinic.

4. Call duty: We have our regular calls on Fridays. An average admission during call was about 10 patients per call.

5. Renal Biopsies: We commenced renal biopsies on our renal patients in April 2005. These were done with ultrasound guidance. We are currently trying to resolve the problems with pathological interpretation of the slides with the relevant department.

6. Dialysis: Residents in the unit undertake regular dialysis in Gbagada.

•  Teaching rounds and seminars: This is also on regular bases for Residents and Medical students posted to the unit.

Facilities available in the department The department has Consultants in virtually all subspecialties of internal medicine. There are up to date equipments to investigate the diseases being encountered in the subspecialties. Endoscopic studies and echo-cardiology examination are undertaken routinely.

Our pulmonary function laboratory has facilities for the evaluation of patients with respiratory diseases. Facilities for Electroencephalography and EMG are also available in our Neurology laboratory. The hospital may also soon be in possession of High Resolution Computed Tomography.

Educational facilities for Residents and Consultants Weekly educational facilities take place in the department. These assist in up grading the knowledge of members of the department and in keeping them abreast of new developments in the practice of Internal medicine. These activities include, Grand Rounds, morning meetings, Journal Club, Joint Medicine/Radiology meeting, and mortality/morbidity meeting, Postgraduate seminars and departmental research forum.

Other Activities Each Unit runs both a subspecialty Clinic and a General Medicine Clinic on weekly basis (see attached Roster).

Patient/Ward Rounds are conducted several times a week by Consultants and resident doctors. Student teaching sessions are run also regularly.
